  • USA-BOX Scorebook Application

    Scorebooks issued by the United States Boxer Association are required for participation in all United States Boxer Association IGP Trials. Only one score book is issued/required for a dog and the score book must be transferred with the dog when and if the dog's ownership changes.
  • To receive a scorebook, fully complete this application, attach all required documentation and submit this form (you will be sent to PayPal to complete the order.)

    Applications must include the following:

    1.  A copy of your current club membership card.

    2.  Copy of the dog’s registration papers if purebred/registered.  If you indicate on the application that the dog is a purebred, a valid registration certificate AKC or an FCI recognized entity must be included with the application.  If that is not included the dod will be identified in the scorebook as a mixed breed.  Dogs do not need to be registered or purebred to be issued a scorebook for working trials.  For a mixed breed or unregistered dog, you need only include the name, sex, and date of birth on the application. 

    3.  Copy of the dog's hip certificate (BK, OFA, or Penn-Hip) if completed (not required to obtain a scorebook). 

    4.  If the dog has already achieved an IGP rating under a recognized judge, proof of each title must be included with the application.

    Current United States Boxer Association Members - $25 per scorebook

    All non-members - $340 per scorebook.

    Replacement of lost scorebooks:

    If your scorebook is lost or destroyed, you must submit a written request for a replacement to the club office along with a $25.00 replacement fee.
