In consideration of participating in an activity sponsored by Asian Counseling and Referral Service (ACRS), I hereby agree to and acknowledge the following:
Release/Liability
I release and hold harmless ACRS, along with its officers, directors, employees, and agents, from any and all liability, responsibility for injuries or illnesses I may sustain, claims, and causes of action arising out of or in any way connected with my participation in the activity.
Assumption of Risk
I understand that there may be inherent risks in participating in this Activity and joining other participants at the activity. I personally assume all risks, foreseen or unforeseen, of injuries and other damages associated with participating in this ACRS-sponsored activity. I further agree to cooperate to the best of my ability to achieve the specific and collective objectives of the activity and at all times follow the specific and general directions given by individuals associated with ACRS. I understand that there is no direct medical health coverage afforded to me during my relationship with ACRS. ACRS is not responsible for any potential exposure to Novel Coronavirus, or COVID-19, which is not a direct result of negligence on the part of its employees, volunteers, or the organization.
Acceptance
I have carefully read and understood this agreement. I am voluntarily signing this Agreement. Before participating, I've had the opportunity to ask questions ACRS about the activity and its preparations, logistical or otherwise.
Publicity
I hereby allow my picture or likeness to appear in any official ACRS document, web and social media postings, public service announcement, and/or news coverage of the activity without compensation to me.
Consent to SMS Message
I hereby allow ACRS to send information via text message to the phone number I provided. ACRS does this as it's one of the most efficient ways to share important information and ensuring proper coordination of personnel in our building is especially critical during COVID-19.
Consent to Conduct Washington State Patrol Background Check
As part of our commitment to creating a safe and welcoming environment, Asian Counseling and Referral Service (ACRS) conducts background checks on individuals who work with vulnerable populations, including children, youth, elders, and individuals with disabilities.
Background checks are conducted through the Washington State Patrol’s WATCH (Washington Access to Criminal History) program and include a review of publicly available conviction and arrest records.
By signing below, you authorize ACRS to request and review your Washington State Patrol background check for the purpose of determining your eligibility to serve as a volunteer or intern.
Storage, Confidentiality, and Access
Your background check results will be securely stored in accordance with ACRS’ data privacy and retention policies. Only authorized personnel directly involved in the review and placement process will have access to this information. Background check data will not be shared with any third party without your written consent, unless required by law.
