I understand and agree that my request to borrow equipment may be denied if I have previously been approved to borrow and/or take equipment off-site from 1300 Kingsdale Avenue, Redondo Beach, CA 90278 but returned past borrowed equipment in bad condition
I promise that the aforementioned equipment will be returned on or before the mentioned date to South Bay Archery Lessons in the same condition in which it was received by the student, if that student is removing it from South Bay Archery Lessons located at 1300 Kingsdale Avenue, Redondo Beach CA 90278
I understand and agree that failure to return the equipment in the same condition may result in a fee charged to me in order to fix or replace any damaged equipment, dependent on the extent of damage.
I understand and agree that I MUST have either Coach Paul, Coach Lani or another authorized range Coach/Instructor sign off on the return of all my equipment BEFORE I am released from guardianship of the borrowed equipment. Failure to do so, may result in the range not knowing you have returned it or other follow-up correspondence requesting return or penalty fee
