Liability Release Waiver
In consideration of the acceptance of my application for I.A.Y.A. Workshop, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to me as a result of participation in said event. This release is intended to discharge in advance I.A.Y.A., its officials, officers and employees, volunteers and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some recreational activities or projects may involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assignees.
Field Trip Waiver
I give my son/daughter permission to accompany I.A.Y.A. on all field trips. I assume all responsibility for risks and hazards incidental for the behavior of my child. I do not hold I.A.Y.A. responsible for any accidents of any kind, or loss of property.
In the event I cannot be reached in an emergency, I hereby give my permission to staff of the I.A.Y.A. Workshop to secure proper medical care for my child as deemed necessary. This permission extends from minor first aid treatment to (under a doctors orders) hospitalization injections, anesthesia, and other medical procedures deemed necessary.
I hereby grant permission to record my child's likeness and/or voice for use by television, films, radio or printed media to further the aims of I.A.Y.A. Workshop in related campaigns and magazine articles, booklets, posters and in any other ways they may seem fit. I give permission for my child to be photographed by I.A.Y.A. either individually or as part of a group participating in camp activities, including offsite trips. These photographs may be used on various camp printed material, internet site, social media, and/or part of a slide presentation to promote participation and interest in the camp.
