Application Instructions
This application is for collecting volunteer preferences for Paizo organized play volunteers at Origins 2018. Information submitted on this form will be shared only with the Organized Play Manager (OPM) and volunteer convention leadership.
Thoroughly read each question before answering. As you fill out the form, other options will open up based upon your selections. For example, when you select which Program to run, the GM Block availability will show. If you select to run 7 or more blocks, the top tier rewards selection will show. Once this form is submitted, you will have access to change your answers but we need that to be done sparingly. Doing so will increases the possibility of errors as we assign GMs to the schedule. All minors (17 and under) require permission to volunteer from their parent/guardian in the form of written (email) permission and will be required to sign the volunteer agreement. Ages 13-17 must have a parent/guardian reachable on site. Applicants under the age of 13 will be considered on a case-by-case basis and if approved must be accompanied by a parent/guardian at all times. Minors earning share of a hotel room must be accompanied by a parent/guardian who is also a qualified volunteer.
NOTE: Completion of this form does NOT indicate acceptance of a volunteer. Emails will be sent directly to each volunteer advising of the status of their registration and require a confirmation response to complete the volunteer process.

The details of each benefit are as follows:
Room—One-quarter of a double room. Volunteers may indicate their roommates preferences when completing the application.
Badge—A free 5 Day Origins 2018 GM badge
*Print Product—A FREE copy of one Paizo print product of your choice valued at $59.99 or less (excludes preorders or backorders).
*T-Shirts—Limited release t-shirts (purple for GMs and orange for support staff) given every other year.
*Store Credit—Paizo.com store credit at $10 per slot
*Boon/Reward—An Origins exclusive boon/reward for Starfinder (RPG), Pathfinder (RPG), or Pathfinder (ACG).
Parking Pass—An in & out parking pass for a neighboring parking lot. A deposit and/or other stipulations may be required.
Other—You may elect for $60 in Origins Vouchers or an additional Origins Convention Badge
