Gymnastics Queensland Athletes' State Team Agreement
(All members of the Queensland State Team are required to complete the State Team Agreement.
This form must be completed by the parent/guardian if the athlete is under the age of 18.)
I hereby agree as a member of the Queensland State Team competing in the 2018 Australian Aerobic Gymnastics Championships from 1st - 3rd May in Bendigo, Melbourne, to travel, stay with, attend, compete and/or participate and perform my duties, fulfil my purpose and conduct myself at all times at the said event, and its associated trials or displays, meetings and official commitments as applicable, under the following terms and conditions:
I AGREE:
(i) To travel to and from the place of assembly of the team or to and from my agreed port of departure upon the dates and by means of conveyance decided upon and/or agreed to by Gymnastics Queensland.
(ii) To stay at the State team accommodation as decided upon by Gymnastics Queensland.
(iii) To pay all appropriate fares, fees, deposits, costs and expenses as invoiced to me by Gymnastics Queensland by the due dates prior to my travel as advised in official notification from the Association and in accordance with the Team Selection document and further agree to pay any costs which may be legally and reasonably incurred by me prior to, during, or subsequent to the event.
(iv) To remain under the authority, management and direction of the Team Manager and/or other Officials as may be applicable from time to time in all matters relating to this event from the date of my acceptance of a position on the Team until my return to my port of departure or official release from the Team.
(v) To conduct myself in a proper manner and to offer myself in a prepared physical and psychological state which will ensure I can exercise to the best of my endeavours the duties, responsibilities and purposes for which I have been selected on the Team.
(vi) Abide by the Gymnastics Queensland Athlete Code of Conduct from the GQ Member Protection Policy Appendix B – Role Specific Codes of Conduct Attachments. (Attachment B3: Athletes Code of Conduct).
(vii) To abide by the ZERO Tolerance Policy of Gymnastics Queensland with regard to the consumption of alcohol, the use of cigarettes and the use of performance enhancing drugs or recreational substances.
(viii) To disclose to the Team Manager and / or other Team Officials any injury or physical or psychological condition which may affect my performance and/or that of my Team, or prevent me from competing or training or being involved at any other occasion which forms part of the event.
(ix) To be responsible for the purchase and wearing of the official competition dress and uniform specified by Gymnastics Queensland at all times as required.
(x) To carry out to the best of my ability all directions given to me by the Team Manager/Coach to ensure the best possible Individual and Team performance and/or coaching or judging standard and/or fulfilment of my responsibilities.
(xi) Not to represent, or purport to represent the opinions, viewpoints, policies, and decisions of the Association to any person in particular the media or official representatives of Gymnastics Australia Ltd, or act in any way on behalf of the Association unless specifically authorised to do so by the CEO or the President of Gymnastics Queensland or the Team Manager or by means of the authority of my position.
I agree to accept that Gymnastics Queensland has expressly provided that in the event of any breach of discipline or disregard on my part of any of the policies relating to Team Selection, I may be subject to one or more of the following penalties:
(i) To be sent home at a time and by means determined by the Team Manager.
(ii) To be excluded from any part of, or all of the competition or event.
(iii) To be suspended or disqualified or banned from future state team representation.
(iv) To remain liable for any costs incurred which may arise as a result of a breach.
I understand that at the time of team departure only athletes who will compete at the Australian Championships will travel and be accommodated with the team. Athletes injured or ill prior to departure will receive a refund less 20% of team travel cost from GQ (except air travel).
Should there be a surplus of funds at the end of the Australian Championships Financial Reconciliation and the surplus amount is more than $100.00 per team member, the amount will be refunded to each team member. If the surplus amount is less than $100.00 per team member the entire amount will be retained and utilised for contingency for the team travelling to following year’s Australian Championships. If Australian Championships are split then the pro rata amount will be allocated to each Australian Championships contingency fund based on which gymsports attend each event.
I have read and hereby agree to abide by the above State Team Agreement and the following GQ By Laws, Policies & Procedures.
