Conviction of a crime is not an automatic bar to employment. SA Life Academy will consider the nature of the offense, the date of the offense, and the relationship between the offense and the position for which you are applying.
SA Life Academy will obtain a criminal history record on all applicants for employment and may contact any law enforcement agency to provide the information. Your typing YES in the box provided at the end of this page acknowledges that you have read and understand that SA Life Academy will inquire into criminal history records. In addition, you understand that if employed, you may be discharged if the SALife Academy obtains information of your conviction for a felony, or any offense involving moral turpitude, that you did not disclose to the SA Life Academy.
Please complete all information below.
Under penalty of perjury, I affirm that the above information is true and correct and I have not knowingly omitted any information requested above. The above data, i.e., race, sex, and birthdate will not be used for employment purposes but is required for identification purposes for the criminal history check. A report showing a record of conviction will not automatically eliminate you from employment consideration or offer of contract, but it may do so. The nature and date of the offense and its relationship to the position for which you are applying will be considered.
