Team Registration Form
All players listed on this roster must submit a signed waiver.
Maximum of 20 players allowed on the roster. All players must be 18 years of age by April 30, 2018.

All players agree to the Rules and Regulations set forth by the Upper Dauphin Co-Ed Softball League, the Dauphin County Parks and Recreation, and the ASA.

Team Fee for 2018 - $565.00 (include ASA team fee, Field rentals, and Umpire fees)

All payments should be mailed to the address listed below. Teams are on a first come, first serve basis. Please fill out and submit the form, and send a copy of it along with the payment. Final rosters will be due by May 5 , 2018.
