  •  Thank you for taking the time to fill out an online hair history and consultation. All these questions help me to understand your needs and better assist you for planning your extension transformation. Great extensions are not a cheap service and proper planning is required to have the best outcome. 

    This form will take approximately 5-10 minutes to complete. Photos are required to be attached here for a proper assessment. Please ensure you have taken 2-3 photos of your hair in natural lighting. 1 photo from the side, 1 photo from the back and 1 photo showing the front of your hair. 

    Pricing estimates will be given after this form is completed. Price varies on brand, quality chosen, method of application, length of extensions and how many packs you need to acheive your goal. Extensions are not a one size fits all service.

    I certify that the information I have provided in the New Client Online Consultation is accurate and true and I hereby indemnify and hold harmless Glam Hair Extensions against loss or damages, claims or actions based on consequences of any and all kinds that I may suffer as a result of my having failed to provide accurate and complete information.

    I agree that Glam Hair Extensions will not be held liable for any loss, damage or injury suffered by me as a consequence of the hair extension service provided to me and I hereby release Glam Hair Extensions from any and all claims or actions that may arise as a consequence of the hair extension service, including but not limited to allergic reactions and hair loss.

    I have read, understand and agree with the hair extension preparation and aftercare information supplied to me on http://www.glamhair.ca/preparation--aftercare.html and I wish to proceed with the hair extension service.

    I have read, understand and agree with Glam Hair Extension's return policy and understand that there is NO REFUNDS on any hair service provided including the hair and install service provided.

    Maintenance is required for a good long term result. Maintenance appointments are suggested at 4-8 weeks depending on multiple factors such as natural hair density, hair growth rate and personal care. At this maintenance appointment, all extensions are removed and reinstalled back up to the root. This time frame will be suggested at your installation appointment and ideally booked at this appointment in advance.

    Tape in extension maintenance is done during two separate visits. A removal appointment and a reinstall appointment. Tape maintenance cannot and will not be done on the same day/same appointment. Handtied Weft/Volume weft maintenance is done in one visit. This is an important point especially for those coming from out of town.

    Any and all items left behind, including hair/hair extensions will be discarded within 2 weeks if not picked up or arranged with me via email.

    Glam Hair Extensions requires a 48h notice for all cancellations and reschedules including consultations. If 24-48h notice is given, a $100 missed/recheduled appointment fee will be added to your future service or deducted from your deposit. If you provide less than 24 hours notice 50% of your service fee will be required prior to rebooking. No shows will be required to pay 100% of the missed appointment value and full payment for the rebooked appointment paid in advance. Any and all deposits are NON-REFUNDABLE, no exceptions. 

    Currently, my new client availability is limited. Weekend and evening availability is limited. 

    Emails, inquiries and DM's on Instagram may not be answered on weekends or holidays. 

    It is recommended to prebook your maintenance appointments in advance to ensure a good time and date for yourself and to stay within the recommended wear guidelines eliminating risk of damage. It is not my responsibility to check in with you to book these appointments. I have a booking form that can be used at your convenience. Alternatively, it is best to prebook future appointments at the end of your visits in person.
