Purpose of the Grant
This grant provides financial assistance to MAHQ members to obtain the Certified Professional in Healthcare Quality (CPHQ) credential or for a current CPHQ to attend an educational conference. Up to two $500 Professional Enhancement Grants will be awarded at the annual meeting in May 2018. Monies awarded for educational conferences are to be used toward travel and registration expenses not reimbursed by an employer.
Open to individuals who are MAHQ members in good standing, excluding members of the current MAHQ Board of Directors.
Applicants seeking assistance to obtain the CPHQ designation should have at least two years’ experience working in the healthcare quality field.
Applicants seeking assistance to attend an educational conference must have the CPHQ credential at the time of grant submission and attest that costs associated with the educational conference are not reimbursable by an employer.
Complete the grant application. Required information must be typed;
hand-written applications will not be accepted.
Provide a personal essay addressing the following:
Reasons for seeking grant. Applicant should also describe perceived benefits for increasing knowledge in QI and/or career advancement.

Applications must be
received by April 30, 2018.
Incomplete applications will not be considered.
Completed applications will be reviewed by the Finance Committee and scored based on the applicant’s compliance with eligibility criteria and quality and comprehensiveness of the essay. The Finance Committee has discretion to recommend recipients to the MAHQ Board of Directors.

Recipients of this grant agree to:
Utilize and seek reimbursement for the grant funds awarded to purchase CPHQ study materials and/or take the CPHQ exam within six (6) months of receiving the grant.
Utilize grant funds awarded to attend the identified educational conference and to attest, at the time of submitting receipts for reimbursement, that expenses are not reimbursable by an employer.
Submit a summary essay (minimum 150 words) to MAHQ about the benefits of receiving the grant within ninety (90) days of taking the exam or attending the conference. Send the statement to
Grant permission to MAHQ to use their name in promotion of the grant program in the MAHQ Newsletter, MAHQ website, at the annual conference, on Facebook and other social media.
