    EMPLOYEE FLEET DRIVER ACKNOWLEDGEMENT

    I have received a copy of the MOHAWK Fleet Handbook and have read the booklet, understand the Policy & Procedures, and agree to comply with its intent.  I understand that violation of the policies within this booklet may result in corrective action, up to and including termination of employment and/or termination from the fleet program.  I understand and agree that in exchange for the various benefits I will receive from the personal use of an MOHAWK leased vehicle, MOHAWK will make a monthly payroll deduction, determined by the vehicle I have selected, to offset the personal use.  I also understand that I am responsible for payment of any traffic, parking and toll violations issued against my Company or Personal vehicle.  MOHAWK will notify the employee Fleet Driver of any tickets or violations paid by the Company and request authorization for MOHAWK to deduct the amount due via Payroll deduction.
