1. Upon submitting the application, we will make camp registration available to you. This registration information will be available online by May 15, 2018 (you will receive a notification of this). The registration form and additional forms must be completed online AND mailed in by July 1, 2018 (latest) in order for your child to attend camp. However, we urge you to submit these additional forms as soon as possible. Again, you will receive additional information for this at the appropriate time. Your acceptance into the program is contingent on completing these additional forms and sending them in by the deadline.

2. We ask that all applicants pay a $175 registration fee that will be used to fund the Life Serve Give Back Program. All applicant fees will be combined into a pool. Through a group-oriented process designed to teach empathy, generosity and consensus-building skills, students will decide together which worthy Columbia County organization(s) should receive this donation.
If the registration fee is unaffordable, as you proceed with the application, you will have the opportunity to request that the registration fee be waived.
*Financial hardship waivers/partial waivers are available for those in need.
3. Life Serve Youth Foundation will arrange transportation to and from camp. Central pick up points for campers will be established for each metropolitan area.
