While we know this form is long, it will help us find the right appointment space for you.  This form helps us learn more about you so that we can customize your appointment.  Please skip any questions you are unsure.The more you answer the better we can help you. Our pricing may be subject to change, please use this as a starting point. The price range indicates Level 1 and Level 6. To see our individual stylists prices please select here.  All first time guests must add on our -Extra Time/New Guest service to allow proper time for a more extensive consultation. A 3$ ECO Fee is applied to every salon service visit to ensure 95% of your beauty waste is recycled.

    DBS Salon is owned by Sandra Moreno, a Redken Certified Colorist, a Deva Advanced Stylist/Ambassador and RëzoCut artist specializing in Curly Hair and Natural texture. We have multiple stylists currently available for services. You can indicate who you wish to see within this form. You can view their work on our instagram @dbs_salon. Please answer the questions to the best of your ability. We love creating a look for you that you are able to recreate at home.  We all work on Curly hair and Natural Textures in varying degrees of experiance. 

    Our online booking for salon services has been suspended for new guests. This questionaire will add you to our cancellation waitlist. Once we reach your name on our list we will text you to notify you of the opportunity to book an appoinment. You may register for the ability to view your appointments and cancel them by sigining up here. As a new guest, we will book extra time on your first visit to allow for us to get to know you and your hair. Our receiptionist will reach out to all new guests who do not submit a photo to ensure proper time is booked.  Filling this form automatically adds you to our waitist.  Most often our wait is 3-6 months unless you select a newer stylist on our team. Occasionally the wait may be less. We get cancellations in 2-3 days in advance and begin contacting those on our list who will fit in the alloted time available. This process can skip ahead in the order of requests possibly getting to you sooner. If we run out of time to fill an appointment we will post the opening in our social media and will book the first who can make the appointment. Be sure to follow us @dbssalon on instagram! 

    In order to ensure that our new guests can book a return visit, we limit advance booking to openings within 12 weeks (3 months) Once you have been seen by a stylist, you are welcome to book your year's worth of appointments with us. 

    To have the best service possible please come with clean, and most importantly, dry hair showing your natural curl/texture that has not been pulled into a pony tail or updo/bun.  We begin every Curly Cut Service with the cut so it is important to have your curls looking their best when you come in. Your hair will be washed and set after your cut.

    We will hold onto your information for 1 year.  After 1 year please resubmit. We do not have an automated system that will text you to tell you that you are still on our list. Thank you for your patience!

  • Please be aware that there is a 48hr cancellation policy.  If you cancel with less than 48 hour notice there will be a 50% fee charged to your credit card.  If you no-call, no-show on the date of your appoitment, you will be charged 100% for your reserved appointment,  billed to your credit card.  We will do everything possible to help you avoid any fees associated with late cancellations by sending you reminders. It is your responsibility to contact us at 408-874-6879 to cancel. ALL FEES are WAIVED if you need to cancel due to illness as long as you notify us at any time prior to your appointment.  If you are sick or feeling unwell contact us immediately, there is no cancellation fees for postponing your appointment when you feel ill. If you come to your appointment with signs of illness, your appointment will be rescheduled and the cancellation fee will apply.

    We require a credit card to reserve your appoitment. It is very easy to create an online account with DBS Salon where you can book, cancel and reschedule your appointments at any time (as long as we have it turned on). Please create your account and add your credit card to reserve your appointment. Please go to DBS Salon Online Booking to create a new account using the email address you provide (or sign in to your existing account) From here, click on the "menu" in the top right corner and then "Credit Cards on File" to add your credit card. Please note, your card may be charged if you do not cancel within our 48 hour cancellation policy as stated above. We hope to provide everyone with contactless payment system at the time of your appointment. If you are having problems creating your account, please ensure that you are not using an auto-generated password and that you are following all password requirements.     If you have chosen to receive a chemical service with us. We pride ourselves on our high level of skill and our long lasting relationships with our guests. Here are some tips that can make your service results more successful:

    1) Chemical services can have varying results based on your individual hair.
    2) It is extremely important that you make your stylist aware of any and all other processes you have used on your
    hair.
    3) Your clothes may be exposed to the chemicals and products despite our best efforts to protect them. If you would prefer, you can dress down into a smock to avoid this risk.
    4) There is a risk of your jewelry (earrings and necklaces) being damaged or lost during your service. We recommend you remove them before your service.
    5) Please feel comfortable contracting DBS Salon at anytime after your service if you have any questions or concerns.

    Liability Waiver

    I have been made aware by a service provider of DBS Salon  (Triptych Studios Inc) that I am choosing to receive a chemical or color service. I have made my service provider aware of all of the processes I have used on my hair in the recent months/years to assist in their assessment of my hair's current status and the process that I need to achieve my desired results. I realize that this is very important information and that any information I withhold regarding my previous processes will increase my chances of damage and the potential for unpredictable chemical reactions. Chemical processes may cause some damage to the integrity of my hair. There are products recommended to improve the health of my hair as well as maintain the results after the service. I am aware that my technician has been trained in the service technique and that they will do their absolute best to create the best results and therefore I will not hold liable DBS Salon (Triptych Studios Inc) or my Service Provider if the process has unexpected or undesired results.

    I understand that there is a required/recommended 48 hour patch test recommended by my service provider and product manufactrurer. By signing this form I agree to not hold liable DBS Salon or my technician if any allergic reaction may occur if I choose to forego this step.
