2019 SUMMER INSTITUTE FOR FACULTY DEVELOPMENT APPLICATION
Please complete the application in its entirety no later than November 30, 2018. At the end of the form, you will be asked to attach your biographical sketch (two paragraphs max), CV, and head shot. You are encouraged to use the CV preferred by your institution for promotion and tenure purposes. Please contact your Associate Dean for a CV template. The application must be approved by your dean/president prior to submission. Please note that if you are accepted as an attendee, you are required to participate in a series of pre- and post-Institute webinars, your biographical information may be shared on the ASCO website and in marketing materials and program materials. Please also note that the SIFD is scheduled to begin at 5:00pm on July 14 and end at 7:00pm on July 17. Please block off July 14-18, 2019 on your calendar.
