Grooming Release Form 2018 - Fur Shore Dog Grooming LLC

I certify that my dog is at least 2 month old, currently up to date on all vaccinations, in good health and has not been sick with any communicable disease in the past 30 days. Fur Shore Dog Grooming LLC will not be responsible if my dog contracts a communicable canine disease from any other dog while in the salon. Fur Shore Dog Grooming does not accept new clients dogs that are over 10+yrs due to senior complications & we highly recommend these senior dogs should be groomed by a veterinarian.

As a responsible pet owner it is my responsibility to use an effective monthly flea and tick preventative.
Fur Shore Dog Grooming LLC does not accept dogs with fleas. If any fleas are found on my dog I understand that my dog will be sent home immediately or require a mandatory Flea/Tick Bath for which I will be charged: $20-40.

I understand that the grooming process may result in the aggravation of pre-existing health conditions or the discovery of new problems or the, (i.e. ear infections, arthritis, skin conditions, etc) especially for senior pets. I hold Fur Shore Dog Grooming LLC harmless to any of these exposures.

I agree to inform Fur Shore Dog Grooming LLC if my dog has ever bitten any human or another dog. I understand that a muzzle may be used on my dog if the groomer feels it is necessary for protection. I understand that if my dog is difficult to handle, requiring extra time to complete grooming, and/or requires more than 1 groomer I will be responsible for paying reasonable “special handling” fees starting at $10. I understand that if my dog is aggressive towards the grooming staff or other dogs in the salon that my dog may be sent home.

I understand that if my dog is matted, I authorize Fur Shore Dog Grooming LLC to de-mat or shave my dog based on the groomer’s evaluation of the matting. Fur Shore Dog Grooming LLC will not de-mat a dog if it seems unfair, uncomfortable or stressful for a dog. If my dog is too matted to be brushed out, I understand that my dog must be shaved to remove the matted hair, and Fur Shore Dog Grooming LLC will inform me if this is the case. I understand that matting often causes skin irritations that I may not currently be aware of, and understand that Fur Shore Dog Grooming LLC is not responsible for any skin irritations that may be revealed or occur due to my dog needing to be shaved very close to the skin. I understand that I am responsible for the additional cost of de-matting or shaving a matted dog starting at $5 per 10 minutes of extra time required, depending on the severity of matting.

Fur Shore Dog Grooming LLC does not use sedatives to groom dogs, nor accepts dogs under sedatives administered at home prior to grooming. I understand that if I choose to sedate my dog there are inherent risks involved when anesthetics and sedatives are administered. I certify that I must inform Fur Shore Dog Grooming LLC if my dog is under any sedative. Fur Shore Dog Grooming LLC has the right to refuse to groom any dog under a sedative, in this case grooming supervised by a veterinarian is recommened in case of any complications.

Fur Shore Dog Grooming LLC uses only pet safe products, however I do understand that it is possible for my dog to develop a skin reaction to a product that may be used. I understand I shall not hold Fur Shore Dog Grooming LLC responsible for any of such occurrences. I will inform Fur Shore Dog Grooming LLC if I know my dog has sensitive skin or issues with certain types of products. I also understand that it is my responsibility to mention if my dog has any open wounds, hot spots or skin infections, as it will be recommended that my dog wait until these conditions are resolved prior to having my dog groomed.

I understand that all charges for services provided by Fur Shore Dog Grooming LLC will be paid in full upon pick-up of my dog. Fur Shore Dog Grooming LLC accepts cash, credit or debit. Fur Shore Dog Grooming LLC reserves the right to refuse service to any customer/dog for any reason. Fur Shore Dog Grooming LLC has a 24 hour cancellation/no show policy to which a $10 fee will apply for missed appointments.

I understand that Fur Shore Dog Grooming LLC will take the utmost safety precautions and care when grooming my dog. I understand and agree that Fur Shore Dog Grooming LLC will NOT be liable for any injury or senior dog's death that may occur in the salon, providing reasonable care and precautions are followed. In the event of an emergency I authorize Fur Shore Dog Grooming LLC to obtain the necessary treatment for my dog and understand that I am responsible for any veterinary costs that may occur. Your dog’s safety and comfort is very important to us at Fur Shore Dog Grooming LLC. Our fully trained staff will do everything possible to ensure your pets safety. However, there is a risk when dealing with animals. If any incident should occur Fur Shore Dog Grooming LLC will contact you immediately.
