Step 1a: Create a Ready Set account
NCH Employee Health Services (EHS) is responsible for managing all information related to employee and volunteer health, including TB and flu vaccines. The EHS database is called ReadySet. As a volunteer, you will need to enroll in ReadySet in order for Employee Health to maintain your volunteer health records. The instructions to enroll are provided below. Once enrolled, your records, reminders and consent forms (called surveys) will be kept there and accessible to you.

Important: When setting up your account please use your legal first name as it would appear on a medical document. Example: Enter Catherine, not Cathy.
Click “New User? Click Here to Begin” and follow the instructions.
You will need the following information:
- Access/organization 4-digit code: 2347
- Program Type: “Volunteer Enrollment”
- Population Type: Choose “New Hire” from list in the system
You will need to save your username and password for future use
Step 1b: Complete the TB & Flu Surveys in Ready Set
While logged into Ready Set, look for the section in the upper left hand corner that says "Health Surveys" and click on this area to complete both the"TB survey" and "Flu Survey" (during flu season).
Each survey status will have a green checkmark under status when you click “Submit Final”.
Once enrolled, continue to Step 2.
