I understand that if I am offered and accept employment with Mid-City Supply Co. Inc. I am required to submit documents verifying my identity and employment eligibility as prescribed by the Immigration Reform and Control Act of 1986. If I fail to comply with the requirements of this legislation, Mid-City Supply Co. Inc. employment offer is automatically rescinded.
I understand that nothing contained in the Employment Application or in the granting of an interview is intended to create an employment contract between Mid-City Supply Co. Inc. and myself.
I will be subject to dismissal or discipline without notice or cause, at the discretion of the employer. I also understand I am free to quit my employment at any time, for any reason, without notice. I understand that the President has authority to change the terms of an at-will employment and that any such change can occur only in writing.
I hereby authorize investigation by the employer, the employer’s associates, agents, or representatives of all information contained in this application, resume, or stated during employment interview(s). I certify that such information is true and understand that misrepresentation or omission of facts called for in this form will disqualify me from consideration of employment or, if employed, will be cause for my immediate termination. I also authorize the employer and/or its associates, agents, or representatives to conduct a background investigation concerning, including but not limited to, my character, general reputation, personal characteristics, education, criminal, and motor vehicle records. I expressly waive any and all rights and claims I might have involving the employer, its associates, agents or representatives as well as all persons, corporations, institutions, or organizations furnishing information about me. Upon written request, additional information concerning the nature and scope of any inquiries made will be provided. I understand that should an employment offer be extended to me and accepted that I will fully adhere to the policies, rules and regulations of employment of the Employer. I further understand that neither the policies, rules, regulations of employment, nor anything said during the interview process shall be deemed to constitute the terms of an implied employment contract.
I understand that this application will remain active for 60 days. If after that time I still wish to be considered for employment, I will need to complete and submit a new application.
I agree to any post offer drug screen and/or physical that may be required by the company.
