MY CHILD(REN) HAVE PERMISSION TO PARTICIPATE IN VBS ON JULY 20 - 24, 2026. BY CLICKING SUBMIT I hereby authorize a representative of St. Margaret Mary Church (SMM) to seek medical treatment for my child(ren) in the event of an emergency in which neither parent nor the adult in whose care the minor has been entrusted can be reached. I authorize any physician or medical center to treat my child. Photo Release-Pictures of my child taken during the event may be used in print or electronic media for the purposes of publicity for future events, unless I indicate to the VBS Coordinator in writing to the contrary. Cancellation Policy-A full refund will be given if canceling before June 1, 2026. No refunds will be made after this date.
