By my above signature, I, the member and spokesperson for ALL members certify and understand that I/we are physically able to use all facilities and do hereby agree that Carter Fitness Holdings, LLC DBA: Ultimate Image Fitness Center (UIFC) and its owners, staff, and independent contractors are not responsible or liable to me for any injury, accident or loss of personal property, whether related to exercise or not. I/we do hereby release this facility and employees from any claim or cause of action which may have occurred due to medical condition known or unknown. I/we understand that I/we cannot transfer this membership to any other person nor is any amount refundable. I/we agree to follow UIFC membership policy and rules and to cooperatively utilize the facilities with other members. Failure to do so may result in termination of my membership. We serve the right to deny access to anyone. Also by signing, I/we opt in to receive communications from UIFC. My email address will not be shared or sold for any reason. I/we certify that I/we have read this agreement and agree to the term herein. Where applicable, EFT Monthly Authorization: I understand that the membership fee will be deducted from my account on the 1st or 15th of each month. I also understand that upon my request to terminate my membership I must give at least 5 DAY NOTICE prior to the 1st or 15th of the month. After the deduction has been processed there will be no refund for the monthly fee under any circumstance. Upon cancellation my access card must be returned 5 days prior to the 1st or 15th of the month or a $5.00 charge may be assessed to my account. I also agree that for any reason if my monthly debit does not go through as approved, it could be collected the next month in addition to the monthly debit and $20.00 returned fee. By signing above I hereby agree to Minor Release (Parent/Guardian) for the Gym as well as Tanning where applicable: I agree to the above signature by minor and agree to above terms. I hereby release this facility and employees from any claim or cause of action which may have occurred due to medical condition known or unknown. I also agree to keep all minors under the age of 13 in the designated children’s area with the understanding that UIFC does not provide childcare. In regards to Tanning, by signing above I am aware that Tanning beds can expose individuals to Ultraviolet radiation, and agree to the rules of tanning no more than once a day for a maximum of 10 minutes. I am also aware that I will need to provide my own eye protection. Furthermore, all memberships including tanning can be revoked by UIFC at any time without notice if non-compliant to facility rules. By signing above, I understand that I have reviewed all rules and guidelines to attend UIFC facilities.
