2) You will need to provide your high school transcripts in JPEG format for this form.

3) Compile a list of accomplishments of competitive ski and/or snowboard competitions

4) Compile a list of activities and accomplishments in school and outside of PASEF activities

5) Provide two letters of recommendation from people who are not family members and are not members of PASEF

6) Be ready to answer a thought-provoking question about yourself.

**IMPORTANT: This form is estimated to take at least one half hour. In order to complete the form as quickly as possible, you WILL need ALL of the above items.**
