The undersigned waive any and all claims or causes of action, we might have against any officer or member of the Board of DACOF, and any person judging this competition. We agree that DACOF has the right to refuse entry for any cause deemed sufficient by DACOF. In consideration of the acceptance of this entry and of the holding of the competition, we agree to hold DACOF, its members, volunteers, directors, governors, officers, agents, superintendents, or competition secretary, equipment providers and the owner or lessor of the show Premises and any employees of the aforementioned parties from any claim of loss or injury which may be alleged to have been caused directly or indirectly to any person or thing by the act of these dogs while in or upon the premises of this competition or grounds or near any entrance thereto, and we further agree to hold the aforementioned parties harmless from any claim for loss of these dogs by disappearance, theft, death or damage or injury, caused or alleged to be caused by the negligence of DACOF or any of the parties aforementioned, or by the negligence of any other person, or any other cause or causes. We hereby assume the sole responsibility for and agree to indemnify and save the aforementioned parties harmless from any and all losses and expenses (including legal fees) by reason of the liability imposed by law upon any of the aforementioned parties for damage because of bodily injuries, including death at any time resulting therefrom, sustained by any person including ourselves on or account of damage to property, arising out of or in consequence of our participation in this competition, howsoever such injuries, death or damage to property may be caused, and whether or not the same may have been caused, or may have been alleged to have been caused by negligence of the aforementioned parties or any of their employees or agents, or any other persons. Furthermore, I agree to be crated in an area designated at the discretion of the Board. Lastly, I understand any unsportsmanlike conduct, as determined by the board, is grounds for dismissal from the competition.
Entry form is not valid and will not be accepted without the original signature of each handler & worker.

All Handlers acknowledge that any dog entered at a Preferred Jump Height has been jumping exclusively at a Preferred Jump Height since 1/23/2023 and is entered at the level of their highest Standard title in EITHER regular or preferred. The penalty for violating the Preferred Eligibility Rules and/or the Competition Level Eligibility Rules will be the disqualification of the dog and the elimination of the dog’s team from being considered in Team Final Standings.
