When collecting a donation, you will be asked to have a photo with the donationtion and our community champion. This photo will be shared on the stores social media and potentailly more widley across Asda, please read the follwoing photography consent form.

To be completed by individual(s) or parents/guardians if subject is less than 18 years of age, before photographs are taken.

As part of our communication Asda uses digital images and film for internal and external publicity purposes. We'd like your permission to digitally photograph/film, you/your property for possible inclusion in our publications, videos, website and other publicity material.

The images/film will remain the property of Asda and will be used for the designated purpose of promoting Asda products, services, aims and values. Your contact details will remain strictly confidential.

I confirm that I am the participant/participants’ parent/guardian/advocate and that I permit Asda and its subsidiaries to use images/video of the named individual(s) in Asda publications and publicity material including its' websites.

I understand that no payment will be made for the use of the images/film.

Your images(s) will be retained on our files. You may withdraw your consent at any time by writing to the Community Team at Asda Head Office, Asda House, Great Wilson Street, Leeds, LS11 5AD (commlife@asda.co.uk).

However, you must acknowledge that images that have been used with your consent may still be in circulation and may not be automatically withdrawn if you leave the business.

Before you submit this form, please understand that our store has a limited community budget available each month that is used for a variety of activities including our school’s activities, instore fundraising and much more.

We try to offer donations to as many groups as possible but due to the large number of requests we can’t always offer a donation. Completing this form allows us to access if we can offer a donation or not and competing the form is not a guarantee or commitment on our part to provide a donation. Donations are capped at £40

All donations are made at the discretion of the community champion in accordance with Asda's donation and ethics policy.
