Parent/Guardian Medical Waiver and Photo Release.

By submitting this form, I hereby release The Rock Church, Laurel, Montana, and its short-term mission teams from any and all claims of liability, personal injury or property damage arising from participation in Sports Camp/Mommy & Me. I understand that participation in sports involves some element of physical contact and I certify that my child is in good health and able to participate in all activities. If any attention is required for illness or injury, I give my permission to a Sports Camp/Mommy & Me staff member for such care.

I authorize The Rock Church to publish photographs taken at this Sports Camp/Mommy & Me of myself and any minor children listed above for use in similar sports camp and mission related promotional purposes, The Rock Church Facebook Page (www.facebook.com/therockmontana), The Rock Church Homepage (www.therockmontana.com), and any other digital or printed promotional purposes for The Rock Church.

I hereby release and hold harmless The Rock Church from any reasonable expectation of privacy or confidentiality for myself and for the minor child and children listed above associated with the photographs specified above. Further, I attest that I am the parent or legal guardian of the child or children listed above and that I have full authority to consent and authorize The Rock Church to use their likeness and names.

I hereby release The Rock Church, its contractors, its employees, and any third parties involved in the creation or publication of marketing materials, from liability for any claims by me or any third party in connection with my participation or the participation of the minor children listed above.

I give consent for my child to be photographed or videotaped during the camp and release use of the images by The Rock Church for similar sports camp and mission related promotional purposes.
