Thank you for agreeing to be an adult reference for an applicant(s) to the National Council of Urban Indian Health (NCUIH)'s Inaugural National Urban Indian Youth and Young Adult Advisory Council (“Youth Council”). NCUIH's Youth Council invites applications from young adult ambassadors (ages 18-24) located in our urban communities.
Youth Council members will promote awareness of substance misuse, trauma, suicide prevention, and mental health among peers on a local and national level.
As part of the Youth Council application process, applicants must submit one (1) mandatory adult recommendation. Please complete the following form and thank you for your willingness to serve as a reference.
