Use this form to submit questions regarding risk management and to report potential violations of Fraternity policies. Please remember that it is important to utilize the resources of chapters, provinces, universities/schools/colleges, and Grand Council Deputies by following the proper chain of command. Complaints and concerns should be addressed with the Regent, GCD, and then the school/college Dean prior to notification of the Central Office. Provinces should notify the Satrap followed by the Supervisor prior to notifying Central Office. Please Note: Anonymous reports may not be investigated as we are often unable to verify claims.

Use this form to bring forth any concerns you may have regarding a violation of our policies which may have occurred at the Chapter, Province, or International level. Please remember that it is important to utilize the resources of chapters, provinces, universities/ schools/ colleges, and Grand Council Deputies by following the proper chain of command. Once an incident is reported, it will be brought to the attention of the Central Office and International Executive Committee.
