The section below regarding the shipment of sale materials only applies to the Girl Scout Cookie Program at this time. This form must be submitted according to the published deadline in order to receive materials shipped directly to you.
If this form is submitted after the deadline, check with your troop leadership to ensure someone else hasn't already received materials. Otherwise, contact Customer Care to arrange for material pickup.

Please be sure that you are communicating with your troop leadership team and only ONE volunteer is receiving materials. If you have not discussed this with your leadership team, please do so before submitting this form!
