  • Fort Worth Abandoned Animal Alliance (FWAAA) is an all-volunteer, non-profit, animal welfare organization dedicated to saving lives and reducing the number of abandoned, homeless, hurt and temporarily displaced animals in the community. FWAAA is completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive complete medical care, which includes worming, vaccinations, spaying/neutering and microchipping prior to their adoption.

  • This questionnaire is designed to help FWAAA determine your preferences and capacity to be a pet foster. All fosters are required to complete this form. FWAAA reserves the right to deny any foster based on answers to this questionnaire and/or interviews

  • Note: If you rent, live in a townhouse, condominium, or apartment, FWAAA will require a written approval letter from your landlord and/or home association.

  • Do you have a secure fence attached to your house, which encloses your yard?*
  • Note: FWAAA requires that all homes have a fenced area attached to the house for safety reasons.

  • Note:  Please list material (wood, vinyl, chain link, etc.)  The fence needs to be a minimum of 5' high (6’ is preferred), and be in good shape so that dog cannot find a way to escape. If you do not know the exact height, please enter an estimate and note that it is an estimate.  An Invisible Fence cannot be used.

  • Transport Foster - A dog that is scheduled to be transported to another rescue out of state. Often times these dogs just need a few weeks out of a shelter environment for state mandated reasons. 

    Foster to Adopt - A dog that stays in a foster home until they are adopted. These fosters will need to attend adoption events and go through our adoption process.

  • Are you familiar with the use of a crate to train and/or confine the dog in your absence or when you are not able to adequately supervise them, to keep him/her out of trouble?*

  • Please know that your application will not be processed when you press the submit button unless you have checked both boxes, below. By checking the boxes you will be agreeing to the following: terms and conditions:

    • That you have carefully reviewed this application and have answered each question truthfully and completely to the best of your knowledge.
    • That failure to complete this form in its entirety and/or knowingly omitting pertinent information may cause your application to be declined.
    • That you understand this application is required in order to foster an animal for FWAAA, but that submitting the completed application is not a guarantee that your application will be approved; it is merely the first step in the fostering process.
    • That you understand FWAAA is a group run exclusively by volunteers who donate their spare time, and, as such, the approval process may take more than a week to complete.
    • That you understand FWAAA a Board Member or Director of FWAAA may request possession of any our foster pets at any time, and you will return our pet upon demand.  This may include taking our foster pet to our vet's clinic to be dropped off or someone of our designation pick up from your possession.  Should your fail to return out pet immediately, it could result in liquidated damages of $2,000.00 in addition to attorney/legal fees.
    • That once personal and veterinary references are approved, a home visit with a FWAAA volunteer is mandatory in order for you to be approved to foster a dog with FWAAA.
    • Should you reside outside the Fort Worth Metro Area, we will not be able to approve your application if we are unable to arrange for a volunteer to conduct a home visit.
    • Lastly, you acknowledge that it is our policy that all family members must be present during the home visit.
  • Photo Release Consent

    We would love to feature you in our marketing media.  This could include our website, Twitter, Facebook, Instagram along with other media platforms.  We need your permission to post the photographs/videos of you interacting with your foster pet and/or our organization.  

    I hereby give permission to Fort Worth Abandoned Animal Alliance to take/use photographs/videos/updates for marketing purposes.  I release and discharge FWAAA from any and all claims arising out of use of the photographs/videos.
