Only fill this out if you've met the dog you wish to adopt and have spoken with Michael or Nicole. Filling this form out doesn't automatically have you adopting one of our dogs. We have to schedule a meet and greet, see if the dog gets along with you and any of your household members and also do a home check*.

*Homechecks are decided on a case to case basis. There are very few circumstances that we won't do a home check, that's usually when a third party we know well vouches for the family or individual.

AS THE ADOPTER, I AGREE TO THE FOLLOWING PROVISIONS:

If the adopted animal is not already altered, I agree to have the adopted animal altered when it becomes 4 months of age.
I agree to keep an identification tag attached to a properly fitted collar that will remain on the adopted animal at all times and to obtain all city licenses required by local authorities. I agree to keep any contact information up to date with the microchip registration company. Louisa's New Leash On Life will complete the initial transfer with the information from this contract.
I agree to provide the adopted animal with regular veterinary care.
I agree to have the adopted animal under my control when he/she is not within the confines of my property. A secure fenced area will be provided for dogs, including shelter from the elements. The adopted animal will not be left tied or chained outside.
If for any reason I can not keep the adopted animal, I agree to notify Louisa's New Leash On Life of the availability of the animal, and to allow them adequate time to help me facilitate finding the animal an approved home. This in no way legally requires Louisa's New Leash On Life to take the animal back. I will never take the animal to a shelter and will not re-home the animal without the consent of Louisa's New Leash On Life.
I agree not to abuse or neglect the adopted animal and I authorize Louisa's New Leash On Life, at their sole discretion to determine whether or not the adopted animal has been abused or neglected.
I understand that the adopted animal covered by this adoption contract is as far as can be deteremined by Louisa's New Leash On Life in good health and that Louisa's New Leash On Life is not responsible for any medical fees incurred after the adoption date.
I agree to give Louisa's New Leash On Life visitation rights to ensure that terms of this adoption agreement are being observed.
I understand that any failure to perform the foregoing agreement will constitute a breach of contract. In the event of any such breach of contract, I authorize Louisa's New Leash On Life to reclaim both possession and ownership of the adopted animal. I also understand that Louisa's New Leash On Life is not liable to me or any other party for any losses, injuries, damages, costs, expenses, lawsuits or judgements whatsoever in connection with my adoption of this animal.

ADOPTION FEE

Adoption fees are different for each dog. If you follow our rescue, you know we take in alot of "behavioral issue" dogs that are bully breeds. These dogs cost us quite a bit of money and time to train and board, and most of these dogs stay in our rescue for many months. We also take medical dogs that need vet care. That is why our adoption fees differ. The "easy to adopt" dogs in our rescue help to off-set costs of the "bully" breeds. All adoption fees are used to pay medical, food, boarding and supply costs. Please make sure you have the correct adoption fee amount for the dog you are adopting before paying the adoption fee. Adoption fees were paid as a "pay it forward" donation.

We may once in a while ask you to pay one of the vet offices we use directly (Advanced Animal Care Hospital, Boca Park Animal Hospital, Cheyenne Tonopah Animal Hospital, Ann Road Animal Hospital, West Russell Animal Hospital, Emergency Veterinary and Critical Care and/or Animal Allergy and Dermotology) or just pay us via PayPal, check or cash. It will depend if our vet bill has ballooned or we are in need of dog/puppy supplies.

We will give you detailed instructions via email after we have received your completed adoption contract how we wish you to pay the adoption fee.

PLEASE NOTE: The adoption fee is a non-refundable donation to help pay for medical costs of the rescue dogs in Louisa's New Leash On Life Rescue.
