Reminder: this form is for priority schedule requests for inpatient medicine services and inpatient consults/electives. This is NOT for clinic, or off-service rotations.

Please keep in mind that inpatient scheduling has limited flexibility by design. I will attempt to accommodate reasonable requests, but there may be circumstances under which I can't grant certain requests. There are several things you can do to make requests more likely to be fulfilled: requesting single days rather than blocks of time if possible, and providing specifics and details to justify your request. The relative priority of each request will be determined by me based on this information and any extenuating factors or circumstances. If you make a reasonable request that can't be granted, I will discuss it with you and attempt to find an acceptable alternative if possible.

There will be a deadline for requests for each month; I will inform everyone via email well in advance of these deadlines. Requests made prior to the deadline are not first-come-first served. Any requests made after the deadline may be denied and/or referred back to you to find your own coverage, as the purpose of this system is to provide you all with a finalized inpatient schedule in a timely fashion.
