Release of Liability and Assumption of Risk Agreement

The risk of loss or injury while volunteering does exist, and while particular rules, equipment, and personal discipline may reduce the risk of serious injury, such chance of injury does exist; and,

I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such the attention of the nearest lead member immediately; and,

I, for myself and on behalf of my heirs, assign personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS the 3E Funding organization, their officers, Officials, agents and/or employees, other volunteers, sponsoring agencies, sponsors, advisers, and, if applicable, owners and lessors of premises used to conduct the event ("Release") WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASES OR OTHERWISE.

Liability Release Agreement* This field is required.
I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT FULLY, UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

VOLUNTEER CANCELLATION POLICY

Cancellation of event attendance must be provided to 3E fundraising 7 days prior to event date and it is the volunteer's responsibility to find a replacement to absolve any cancellation fees.

Cancellations must be submitted to Lisa Onks 931-624-4674 or Melanie Holcomb 6859-391-2742

Failure to provide Cancellation of event and /or replacement for attendance will result in a cancellation fee of the minimum event donation amount assessed to the registered volunteer's profile account. Again, Cancellation fee will not be assessed if a substitute for the original registered volunteer is provided at the event.

Non-attendance of an event without cancellation notification, will result in a cancellation fee of the minimum event donation amount assessed to the registered volunteer's profile account.
