This form authorizes Legend Rings to charge the above filled in amount to the individual named on the form. Due to the custom nature of this product, we do not accept returns. However it is our goal to make all of our customers satisfied. Please ensure the spelling of Name, Number and/or Position or Title is correct before submitting as this is what will be put on your ring. If you are unhappy in any way, please let us know and we will do everything possible to fix it. Your ring will take approx. 10 weeks to complete and be ready for delivery after the order deadline.
