This Agreement is made effective for all purposes in all respects as of {date} by and between Kate Doherty Images, hereinafter referred to as "the COMPANY" and {name}, hereinafter referred to as "The CLIENT" relating to the event(s) detailed below, hereinafter referred to as "The EVENT(S).

 

I, the CLIENT,  hereby grant to the COMPANY,  the right and permission to use my name, likeness, image, and/or appearance or my minor child as embodied in photographs of me that were taken by the COMAPNY (my LIKENESS), during the EVENT agreed as a photography session.

I agree that the COMPANY may, but has no obligation to, use my LIKENESS for any purpose consistent with the promotion of  business  including the display of my LIKENESS on business website and social media pages, and to re-use, publish and republish the same in whole or in part, separately or in conjunction with other photographs, in any medium now or hereafter known for any purposes whatsoever, including illustration promotion, advertising and trade.

I, the CLIENT, release and discharge the COMPANY from all and any claims and demands ensuing from or in connection with the use of my LIKENESS, including any and all claims for libel and invasion of privacy. I acknowledge that I will not receive any compensation for the use of my LIKENESS.

This agreement is being made and entered into under the laws of the Commonwealth of Massachusetts and shall be governed and interpreted in accordance with its laws.

I, the CLIENT,  represent that I am over the age of eighteen (18) years and that I have read the foregoing and fully understand its contents.* 

*I hereby certify that I am the parent or legal guardian of {name}, named above, and do hereby give my consent without reservation to the foregoing on behalf of this individual.
