To sign up for GO TEAM Training, please complete all required questions below. Upon successful completion of GO TEAM Training and passing the examination, you will be offered a one year membership in the GO TEAM, renewable annually.
Before you can apply, you and your dog must receive the recommendation of an SMS Franchise Owner, a GO TEAM approved evaluator, or other GO TEAM approved Dog Training Program. We do contact all evaluators to verify each handler/dog team class recommendation.
To make the signup process go quicker, please have the following available before you begin:

To complete this application, you will be asked to electronically sign an acknowledgement using a commercial tool called Docusign. How long will this take?
Less than 15 min (estimated).
I'm attending as a second handler. What do I need to do differently?
Just check the box for "Applying as an additional handler in the same household" and we will pair your application with the primary handler. We will not re-ask for the dog's information.
Do you handle my information securely?
Yes. We are using a third party site (www.JotForm.com) to collect information via their secure website (https) and deliver it to us securely for our records.
How does PayPal work?
Once you press submit at the very end, this form will redirect you to www.paypal.com to complete the payment. You do not need a PayPal account to pay by credit/debit card.
