  • If the Bishop is expected to be at two services on a Sunday, please list out the second service time below. Otherwise, skip to the next section.

  • If there are persons to be presented for confirmation, reception, reaffirmation or baptism, please input the number of people below.  Please insert 0 for the categories with no persons and move to the next section.  

  • Please note, if there are any persons to be baptized, confirmed, received or reaffirmed who identify as non-binary, please provide the Bishop with the person's name and the pronouns they wish to be referred to with.  The Bishop likes this information as a matter of courtesy, but also because the Bishop uses pronouns during the prayer over the person when laying on of hands. 

  • Worship Service Information

  • The readings used for the service should that day's appointed readings from the lectionary.  Since, often, there are optional first lesson and psalm, please list out the readings for the service. You do not need to input the full reading, just the item/page number from the BCP for the Collect and Psalm and the lesson/Gospel chapter and verse(s). If you are interested in using readings other than those appointed for that day, please list out the readings and the rationale in the last box.

  • Other Information

  • Do you want the Bishop to participate in anything else, such as Adult Forum or other group gatherings?
  • Will the Bishop have an opportunity to meet with the confirmands, baptismal candidates and/or sponsors?
  • Will the Bishop have an opportunity to meet privately with the clergy?
  • Will there be a reception or meal after the service?
  • Will there be other social gatherings you would like the Bishop to attend?

  • The Bishop's office is attempting to get pictures from each of his visitations to put on the diocesan website, Facebook page and include in the monthly newsletter.  Please have someone take pictures that day.  Cell phone pictures are fine as most phones have high resolution cameras in them.  The Visitation Reporting Form will have a place the pictures can be uploaded or they can be emailed to dnwtcomm@nwtdiocese.org.
