Separately submit the following:
1. Transcripts of academic record from high school, college, university or other institution if you graduated in the last 10 years.
2. Letter of verification from the URG member company. This letter should be on company letterhead signed by the owner, where the applicant or the parent of the applicant is currently employed and in good standing with the employer.
3. Completed Certification Form. This form must be hand-signed by the employed parent or the applicant employee. There are separate forms for each. Please make sure you are selecting the correct one.
4. Letter of Recommendation (OPTIONAL) from a mentor, teacher, or employer that supports you.
