The children listed on this form have my permission to attend and participate in Vacation Bible School at Faith Church. I release Faith Church and its volunteers/employees from all liability for any injury that may occur while on the premises at this time. I hereby authorize Faith Church to administer basic First Aid to these children and to seek professional medical treatment if necessary.
