Includes: • Custom nutrition plan with updates • Custom macros with updates • Macro counting guide • Access to the Cuong Strong Nutrition course • Training plan • Unlimited text support Cost: $170 for the first month then $120 for each month after (sales tax included). Payment is due at the time of this contract and monthly subscription dues will be automatically charged to the debit/credit card on file. Optional upgrade: weekly coaching calls for an additional $100 per month or ultrasound body composition assessments for $20. Refund Policy: Personal training & nutrition results may vary depending on the individual. Nutrition plans are NOT DIETS; they require a commitment to a lifestyle change. Because we only choose to work with committed clients, there is no refund if you are unable or unwilling to follow our recommendations. You may gift or sell your training sessions to another client or use them at a later time as they have no expiration date. Subscriptions can not be transferred or used at a later date.

By checking this box and signing below I agree to the following terms: I am aware of my responsibility to consult with my personal physician regarding my clearance to engage in strenuous exercise and/or a nutritional support program. I acknowledge that the Cuong Strong Gym is an unsupervised fitness center and I assume all risks associated with using exercise equipment and exercising alone without the aid and presence of staff on the premises. Cuong Strong LLC may use photographs of me or my image with or without my name for marketing purposes such as advertising, social media posts, and website content. I do hereby intend to be legally bound for myself and waive release of any and all rights and claims for damages I may have against the participating training facility and the fitness trainer/certified fitness nutrition specialist administering this program as well as the program creators themselves or anyone in connection with them for any and all injuries suffered while following the training and/or nutrition program provided to me.
