    I hereby select XBLUE Networks, LLC, to be my local exchange provider and to act as our agent in dealing with our current local exchange telephone company. XBLUE Networks, may place orders for new services, changes to existing services, and request and receive the results of busy/traffic studies. This authorization covers the following locations and lead billing telephone numbers and shall remain in effect until further written notice is provided. I understand that I can have only one local service provider for any one telephone number.

  • DO NOT USE PO BOX AS ADDRESS. CARRIERS WILL REJECT IT. 

    • If your bill has PO Box Address, please use your Service Location instead.

    Please Upload or FAX your most recent phone bill and ensure the following:

    • Bill MUST include ALL PAGES
    • Account # & PIN # (if applicable) are CLEARLY VISIBLE (Handwrite PIN on bill if not included)
    • All Phone number's MUST be listed on the phone bill, including numbers that are not porting (Especially true for partial ports)

    Note: Please be sure to match the "Service Address" on your current carrier phone bill in the form address area.

  • PORTING PROCESS ADVISORY:

    Before Submitting:

    • Before the porting process begins please ensure you have forwarded your numbers to the temporary numbers that XBLUE has provided (in your Welcome Letter). If you are not sure give us a call @ 866-925-8312

    After Submission:

    • Porting can happen instantly or take up to 40 days
    • The date selected for a port (by your current carrier) is the earliest possible port date that they can accept (due to their readiness)
    • The date that they select will be the first date that the port can take place, but is not a guarantee that it will take place on that day

    Reversing a Port:

    • If you want to cancel a port that is already completed and move them back to the old carrier (this is called a snapback) Heavy fines are accessed. There is a 24hr or less window for this to occur after which time the snapback is no longer possible.

  • Need help picking a submission date?

    The date you input here is the earliest possible date that you wish your Port to take place. The port of your telephone number(s) can take place on this date or up to 40 days thereafter. It is important that we don't submit your port request until you are ready for it to occur. Use the following points as a guide to the date you'll request:

    • Your new phone system should be in place and fully functional before the date entered.
      • Know when you will receive your new XBLUE phone system. (XBLUE will send you tracking information.)
      • Plan the installation.
      • XBLUE provides you with temporary telephone numbers to allow installing your new phone system; the temporary numbers can be used to direct calls to your new XBLUE phone system while you wait for your port to complete. You can use the call forward feature on your current carrier to forward calls to the XBLUE temporary number - thereby allowing you to receive calls on your new phone system from callers who dial the number you are porting.
    • Your existing carrier will advise XBLUE of the actual port date after they receive the Port Request.
    • XBLUE will then pass to you the Port Date set by your current carrier. This is called the FOC (Firm Order Commitement) Date.
    • You MUST NOT cancel your service agreement with your old carrier until the Port is COMPLETE. (This may happen very shortly after (immediate in some cases) the submission date or take up to 40 days after the submission date.
    • You will be provided a tracking number for your XBLUE phone system order after it's processed, this may help you to determine your timeline. You may want wait for that information before selecting a date and/or submiting this form.
  • Certification

    I certify that I have read and understand the letter of authorization and the terms and conditions, I further certify that I am at least eighteen years of age and that I am authorized to change telephone companies for services to the telephone numbers listed above. I authorize XBLUE Networks to act as my agent to notify my local phone company of my decision to change my current long distance service to XBLUE Networks service. I understand that my local phone company may charge me a fee to switch long distance carrier(s). Selection of XBLUE Networks will apply to the telephone number(s) listed on this form. I, the customer, understand that I may designate only one inter-exchange carrier for one telephone number for interLATA and, when applicable, intraLATA usage, and herby designated XBLUE Networks as my primary carrier.
