• INP Surveillance System Evaluation Tool

  • The CORDS Inter-Network project on digital event information and data collection aims to empower communities and enhance cross-border health security by improving digital community detection and response to One Health priorities, in cross-border Network areas.

    The information collected using the Inter-Network Project (INP) Surveillance Evaluation Tool will be used to assess current early warning and response (EWAR) systems and practices in human and animal health events from cross-border communities, in each Network.

    The INP Surveillance Evaluation Tool consists of this 10 page assessment, completion of a separate Event Log, and submission of any supporting documents. it is recommended that the Event Log and supporting files are prepared first- as they can be uploaded directly through page 9 and 10 of this form.

  • INSTRUCTIONS:

    The following section is intended to capture key information about recent public health events reported to your existing surveillance system from official and unofficial sources.

    If you have an existing database of surveillance events:

    If you already have a digital version of this information recorded on a spreadsheet or a database which can extract to data to .csv or .xls formats; it will be easier for you to submit your event reporting data the following way:

    • Carefully read the definitions for the required data points described in the event log instructions to ensure they are consistent with those used by your local system.
    • Extract data relating to the required data points from your database to either a .csv or .xml file format.
    • Delete any other columns/ data points other than those required. 
    • If you are extracting data directly from a database please provide records for all events recorded over the past 12 months. If your database contains less than 12 months of records, please provide whatever you do have.
    • Submit your .csv or .xml file using the "Browse Files to Upload Data Set" button below.

    If you do not have an electronic database of surveillance events:

    • Download a copy of the Event Log template and instructions HERE
    • Input the required information into the Event Log Template from your local paper based records or other event recording system.
    • Submit your completed Event Log template using the "Browse Files to Upload Data Set" button below.
