Through this form, South Valley Community Church (SVCC) discloses to you that an investigative consumer report is being obtained from a consumer reporting agency (Protect My Ministry) for the purpose of evaluating you for employment, for retention as an employee, or as a volunteer. This report may contain information bearing on your character, general reputation, personal characteristics, or mode of living from public record sources. You may also have a right to request additional disclosures regarding the nature and scope of this investigation.

Authorization to Conduct Background Investigations

Thank you for your interest in volunteering for service with the Children's Ministry at South Valley Community Church (​"SVCC”). We take the safety of the children left in our care very seriously. Children's Ministry volunteers at SVCC must submit to various screening and child safety training procedures. The primary screening tools used for youth under the age of 18 are personal interviews and personal reference checks. The results of all inquiries are kept in strictest confidence. Individuals who have been arrested, charged with, on probation for, or have been convicted of sexually oriented crimes, or other crimes against children are not permitted to serve in any capacity within SVCC's Children's Ministry. Notice is hereby given that ​our screening process will be conducted for ​all volunteers prior to their approval to serve in SVCC's Children's Ministry. The screening process may reveal information about your character, general reputation, personal characteristics and mode of living, ​where applicable. They may involve personal interviews with sources such as your neighbors, friends or associates. The scope of this notice and below authorization is not limited to the present and and may continue throughout the course of your ministry tenure thereby allowing SVCC to conduct future screenings, as permitted by law and unless revoked by you in writing. By my approval (selecting "Yes") below I hereby authorize SVCC staff to conduct personal reference checks for the purpose of verifying my suitability for serving in SVCC's Children's Ministry at any time after receipt of this authorization and throughout the course of my ​service in SVCC's ministry, if applicable.

These reports may contain information about your character, general reputation, personal characteristics and mode of living, whichever are applicable.  They may involve personal interviews with sources such as your neighbors, friends or associates. The reports may also contain information about you relating to your criminal history, credit history, driving and/or motor vehicle records, education or employment history, or other background checks. 

You have the right, upon written request made within a reasonable time after the receipt of this notice, to request disclosure of the nature and scope of any investigative consumer report prepared by contacting the Company and Protect My Ministry 14499 N. Dale Mabry Hwy., Suite 201 South, Tampa, FL 33618; Phone: 1-800-319-5581.  For information about Protect My Ministry’s privacy practices, see www.protectmyministry.com. The scope of this notice and below authorization is not limited to the present and, if you are hired, will continue throughout the course of your employment and allow the Company to conduct future screenings for retention, promotion or reassignment, as permitted by law and unless revoked by you in writing.
