After you watch the video, please answer the questions below the best you can. Send your answers back to the church office (or give to the pastor) 2-3 weeks prior to the date of your baptism. We must receive this questionnaire back in order for you to be approved as a candidate for baptism.

To help you with the questions below, go to your Bible and read these verses from the New Testament: Matthew 28:18-20; Romans 6:1-4; and Ephesians 2:8-9
