BEFORE YOU GET STARTED, this form is designed to collect as much information as possible and is separated into 4 sections:
- Section 1: You, the Main Contact Person
- Section 2: Your Organization or Cause
- Section 3: Your Event, Project or Campaign
- Section 4: Your Request
You will be able to attach documents (e.g. letters, posters, images, etc.) with your request. You can aslo save your progress to return at any time -- see the 'Save' icon at the bottom -- which will provide you a link to use to resume your work or share (at your discretion) to have others review or fill it out for you.
A PDF summary will be attached along with a confimation number (by e-mail) once you submit the completed form.
We recognize the growing needs of our communities but do ask for your understanding that we may not be able to support every group or worthwhile cause.
IMPORTANT: A minimum of 10 working days is needed for the review of your request.
