Birthday Party Request Form

Please use this form to submit a birthday party request. Don't see the time you want? Give us a call and we'd be happy to see if your time fits within our availability.

Northern Plains Dance will contact you to finalize the date and time of your party. All rentals are scheduled based on instructor and studio availability. Payment of the Base Fee is required in order to reserve your rental (paid after NPD confirms your request).

Final guest count and payment for any upgrades is required 2 business days prior to the party date. The Base fee is for a party of up to 10 children. NPD requires 1 chaperone for every 10 children. Additional adults are not permitted.
