I acknowledge receipt of company provided uniform:
I understand that I am responsible for the company provided uniforms. It is my responsibility to maintain clean and presentable uniforms. I am to report any wear and tear conditions to my manager for replacement. Damage caused to the clothing, either from abuse, misuse or lost items must be reported to my manager. Failure to comply may be subject to disciplinary action.
In the event of employee termination, uniforms are to be returned as part of the exit process, to my manager.
