    Wellness Assessment
  • This self-assessment, created by Healthy Seminarians - Healthy Church (HSHC), is intended for your personal use in self-reflection and goal setting. The information is not intended as medical or emotional health advice or for diagnostic purposes.

  • Your answers to the assessment will be kept confidential and not shared with anyone in association with your personal identification.  It will only be used by HSHC and combined  with other people’s information to create programs for seminarians, clergy, and congregations that encourage wellness.

  • Instructions:

    At the end of this assessment, you will receive a score based on your responses compared to the total possible score in each of the four areas of wellness: emotional, physical, spiritual, and mental.  If there are areas where you wish to create goals, a SMART goal setting planner is provided at the end of the assessment, along with resources to support your wellness goals.

    Please rate the following statements regarding your health and wellness based on whether you Strongly Agree, Agree, Somewhat Agree, or Strongly Disagree.

  • Emotional Wellness

  • 1. I love and accept myself as I am.
  • 2. I have loving, supportive relationships (friends, family) in my life.
  • 3. I maintain a balance of my needs and the needs of work, family, friends, and other obligations.
  • 4. When I experience positive or negative emotions, I can appropriately express how I feel.
  • 5. I am resilient and can bounce back after a disappointment or problem by focusing on what I can learn from the event or situation, or what positive consequences may result.
  • 6. I seek help from others when I am having trouble with life or health (doctor, counselor, spiritual director, etc.).
  • 7. I regularly take steps to manage the effects of stress (yoga, exercise, times of quiet, meditation).
  • Physical Wellness

  • 1. I get 6-8 hours of sleep and wake feeling refreshed and rested.
  • 2. I engage in physical activity 3 or more days each week (e.g., walking, yoga, cycling, swimming, running).
  • 3.  I am mindful of staying hydrated throughout the day.
  • 4. I eat a healthy, balanced diet of fruit, vegetables, and whole grains and limit consumption of processed and fast food, animal products, sugar, salt, and caffeine.
  • 5. I either do not drink alcohol at all or drink alcohol in moderation (it does not affect my ability to perform daily activities).
  • 6. I do not use recreational drugs.
  • 7. I do not use nicotine.
  • 8. I am satisfied with my body.
  • Mental Wellness

  • 1. I regularly engage in stimulating or creative activities.
  • 2. I regularly engage in continual learning opportunities and am willing to re-examine what I believe considering new information.
  • 3. I enjoy learning about subjects other than those I am required to study in my field of work.
  • 4. I fully immerse myself in tasks with energized focus and enjoyment.
  • 5. I engage in downtime or relaxation that lacks specific goals or focus in order to recharge my brain.
  • 6. I am satisfied with the current state of my finances and am intentional about how I spend my money.
  • Spiritual Wellness

  • 1. I spend time evaluating what I feel is important in life (e.g., who I am, what I value, where I fit in, my future direction).
  • 2. I try to live each day in a way that is consistent with my values.
  • 3. I feel that my life has a sense of direction or meaning to it.
  • 4 . I accept events and others as they are and do not make judgments.
  • 5. I feel a sense of connectedness with something greater than myself, creation, and all other living beings.
  • 6.  I regularly follow practices for spiritual growth (e.g., prayer, meditation, spiritual direction).
  • 7. I have found, or am seeking to find, a local worshiping community.
  • 8. My work/studies reflect my personal values.
  • 9. The work/studies I am pursuing do/will accomplish something important.
  • 10. I believe that I can contribute my knowledge, skills, and talents in my work/study environment.
  • 11. I am currently volunteering, or would like to find the opportunity to volunteer, in ways that use my knowledge, skills, and talents constructively.
  • 12. I currently feel very clear about my “call” or vocational direction.
  • 13. I feel confident that I am acquiring the skills to achieve my vocational goals.
  • 14. I seek opportunities to learn practical skills to help others.
  • 15. I respect the diversity as well as the commonalities of all people (e.g., race, ethnicity, religion, gender, ability, or sexual orientation) and work to promote acceptance for all within my community.
  • 16. I contribute towards making my environment a safer and healthier place for all people (e.g., recycle, use eco-friendly products, etc.).
  • 17. I participate in a wide variety of social activities and find opportunities to form new relationships.
  • 18. I stay informed about local, national, and/or world events.
  • 19. I recognize and work to minimize the impact of the environment (both natural and social) on my health.
