Registration is a simple three-step process and completely online:
STEP 1:
Complete this form (Client Profile & Scope of Service). This gives us detailed information on your agency and how you would like to utilize our services.
STEP 2:
Upon completion of this form you will be re-directed to our contract which you will sign via DocuSign eSignature service (you do not need a DocuSign account to complete the contract) and submit the one-time agency set-up fee of $450.00 via ACH (Automated Clearing House).
