This form is to be processed when a student transfers from one school to another without a corresponding change of legal residence of both the student and the parent/guardian and wishes to participate in interscholastic athletics (MPA Bylaws, Article III, Section 4).
Is the student entering high school as a ninth grade student?

Yes - If YES, stop here. This form is NOT necessary.No - If NO, please continue.
Was there a bona fide corresponding change of address by both the student and their parent/guardian?

Yes - If YES, stop here. This form is NOT necessary.No - If NO, this Transfer Waiver Form must be completed and submitted.
