The above named company is applying to participate in the JCYM 2024 Holiday Boutique and will meet the following:

Attendance on ALL days & times is required Tuesday, December 3rd from 1:00-9:00 pm and Wednesday, December 4th from 9:00 am-7:00 pm.
Booth set-up will be Tuesday, December 3rd from 8:00-12:30
I am paying/mailing my space rental fee and agree that 15% of gross sales (excluding sales tax & shipping) for these two days will be donated to Johnson County Young Matrons, Inc. (a Not-For-Profit Corporation, #48-0721509).
I understand that JCYM will provide a tax donation letter for my contribution.
Every effort will be made to address your special considerations but no guarantees are made
Location of booth space is at the discretion of the Boutique Chairs
I understand JCYM will notify me if my application is/is not accepted by JCYM.There will be absolutely no refunds unless JCYM does not accept the above application.
All federal, state, and local laws governing retail sales tax must be followed.
