For Residents of Pennsylvania, 10 years or more

Please note that for people who have resided in Pennsylvania for 10 consecutive years or more can fill out the residency affidavit and do not need to get FBI fingerprinting. If you sign the residency waiver you will be required to consent to a National Background Clearance, the office will be contacting you with additional details. If you are not a PA resident, or lived in PA for less than 10 years, this residency affidavit is not applicable. Please proceed to the Fingerprinting Directions. 

If you are choosing to do the Residency Affidavit instead of Fingerprints you will need to have a National Clearance at no cost to you. Click HERE and fill it out requesting a National Clearance consent form. Once I get your information, you will be receiving an email from message@mobilizemyministry click the link in the email and fill out the required information.

Fingerprinting appointments can be set up at https://www.identogo.com/

1. Click on "Get Fingerprinted" at the top right corner.
   • Select A Fingerprinting Service by State - "Pennsylvania"
   • Enrollment Services - "Digital Fingerprinting"
   • Service Code - 1KG6ZJ (DHS Volunteer)
2. Choose "Schedule or Manage Appointment."
3. Enter your personal information as prompted, then click "Next."
   • Employer information: Covenant Fellowship Church
4. Choose and appointment date and time.
5. Review the submitted information and click "Done" when finished. 
6. The cost is $23.25. Keep a copy of your receipt. 
7. Go to the chosen Identogo location to have your fingerprints taken. Results will be emailed to you.
