1. Safety application can be downloaded and printed HERE. 
2. The completed application can be submitted below or sent to the church office to the attention of Lori Snyder. (Requires parent signature.)

1. Please send the reference link to three references.
   1. One professional reference
   2. One personal reference
   3. One family member
2. References must include one family member and one member of the opposite gender and can include members of the church.
3. Reponses to the reference form will be received and retained by an authorized CFC staff member and handled by them and the pastoral team as appropriate.

1. Once you have received the email, click on the training link within the email.
2. You will be directed to a webpage with videos of the MinistrySafe training. The entire training is an hour and a half long and is broken into nine 5-10 minute videos.
3. Upon completion of all the videos, there is a short quiz to complete the course. 
4. A copy of your certificate will automatically be emailed to the church office. Please keep a copy of your records.
